Cervical Pillow: What It Is and How to Choose One
A cervical pillow is a pillow shaped to support the natural curve of your neck while you sleep. Whether it helps depends mostly on one thing: whether its height fits your shoulder width, neck and sleeping position.
Key takeaways
- A cervical pillow only supports your neck if its height matches your body. A pillow that is too high or too low leaves the neck tilted for hours.
- A cervical pillow is an adjunct to care, not a treatment for a neck condition. If you have ongoing pain, numbness or tingling, see your own provider.
- Pillowise uses three measurements (shoulder width, neck circumference and neck height), plus sleeping position and mattress firmness, to pick one of 7 sizes. The measurement is free at a provider near you.
What is a cervical pillow?
The cervical spine is the seven vertebrae of your neck. A cervical pillow is built to keep that part of the spine supported while you lie down, instead of letting the head drop toward the mattress or push up and away from it.
You will see many shapes on the market: contoured, wedge, rolled, flat and adjustable. The label "cervical" is not a standard. It tells you the pillow is meant for the neck. It does not tell you whether it fits your neck.
That is why we start with height instead of shape. The head, the neck and the shoulder together decide how much space the pillow has to fill.
Does a cervical pillow help with neck pain?
It can be one part of the picture, but we do not present it as a treatment. Neck pain has many causes, and a pillow does not address most of them.
What the research shows so far is limited. In a pilot EMG study of 15 adults, a pillow height matched to shoulder width produced the lowest neck and shoulder muscle activity in side sleeping (Jiao et al., 2025). The study was small, it enrolled healthy adults and it measured muscle activity, not pain. It shows an association with comfort and muscle load. It does not show that a pillow cures anything.
For a closer look at the evidence, read our summary of adjusting pillow height for neck pain in clinical studies. If you already see a chiropractor or physical therapist, this post explains where a pillow fits alongside care.
Why does one pillow height not fit everyone?
A pillow is like a shoe. One size does not suit every foot. Several factors change how high the pillow needs to be:
- Shoulder width. On your side, the shoulder is the widest point between your head and the mattress. A wider shoulder needs more height. See side sleeping, shoulder width and pillow height.
- Neck size and length. A longer or thicker neck changes the gap the pillow has to fill.
- Sleeping position. Side, back and stomach sleepers need different heights. Your sleeping position is the one you fall asleep in.
- Mattress firmness. On a soft mattress you sink in, so the pillow can be lower. On a firm mattress it usually needs more height.
- Body weight. A heavier person sinks deeper into the mattress and may need a lower pillow.
For a walk through position and mattress combinations, see how to choose a pillow based on your sleep position and mattress. We explain our reasoning in more detail on why a fitted pillow makes more sense than a stock height.
How do you know if your pillow is the wrong height?
Common signs are waking up with a stiff neck, bunching or folding the pillow to change its height, or shifting position all night to get comfortable. None of these proves the pillow is the cause. They are a reason to check. Our guide on how to tell if your pillow is the wrong height lists what to look for.
How does Pillowise approach the cervical pillow?
Pillowise picks the pillow size from your body, not from a guess. A provider takes three measurements:
- Shoulder width. This sets the gap for side sleepers.
- Neck circumference. Measured at C4/C5, which is below the Adam's apple for men.
- Neck height. Measured from the mastoid process (the bony bump behind your ear) diagonally to the outer edge of the acromion (the top of the shoulder).
The provider then adds your sleeping position (side, back or stomach) and your mattress firmness (soft, medium or firm). If you do not know your firmness, we use medium. If you are unsure of your position, we use side. The Pillowise algorithm combines these five factors and selects one of 7 sizes. Two people with very different bodies can end up with the same size.
The system indicates the right size for about 95% of people. Weight is not part of the calculation, so the provider checks how you lie on the pillow and adjusts if needed, for example during pregnancy. A second person taking the measurements helps. You can see the manual method on our measurement page, and this post describes what happens in a fitting.

What if the pillow does not feel right?
Give any new pillow 7 to 10 days to adjust. Your neck is used to the old one. After that, you can exchange it for another size or return it for a refund within 30 days of purchase. You pay return shipping, and shipping charges are not refunded. The pillow also has a 5-year warranty on material and production faults. It does not cover normal changes in firmness, improper use, wear and tear, contamination or damage after delivery.
When will a cervical pillow not help?
A pillow cannot fix a problem that does not come from sleep position. See your own provider first if you have:
- Numbness, tingling or weakness in an arm or hand
- Neck pain after a fall, accident or sports injury
- Pain that keeps getting worse or does not ease over time
Poor sleep itself can also make pain feel worse, as we describe in does poor sleep make neck and back pain worse. A pillow is one small change within a larger plan.
