What Is an Orthopedic Pillow, and Do You Need One?
An orthopedic pillow is a pillow marketed for neck support, usually with a contoured shape or memory foam. The label does not tell you whether it fits your body, and fit is what decides if a pillow supports your neck.
Key takeaways
- "Orthopedic" describes how a pillow is marketed. It does not tell you the right height for your shoulders and neck.
- Shoulder width, neck size, sleeping position and mattress firmness all change the height you need.
- A pillow is an adjunct to care, not a treatment. If you wake up with ongoing pain, see your own provider.
Does an orthopedic pillow fit everyone?
No. A pillow should fit the body, like a shoe size. One stock height cannot suit every shoulder width, neck and sleeping position.
A side sleeper needs a pillow that fills the gap between the shoulder and the head. A back sleeper needs less height, and a stomach sleeper needs the least. A soft mattress lets the body sink in, while a firm one does not. A pillow labeled orthopedic can be too high or too low for you, like any other pillow.
The research points the same way, with limits. In a pilot EMG study of 15 adults, a pillow height matched to shoulder width produced the lowest neck and shoulder muscle activity in side sleeping (Jiao et al., 2025). The sample was small and did not include people with neck pain, so it shows an association, not a cure. We cover it in side sleeping, shoulder width and pillow height.
How do I know if my pillow is the wrong height?
Common signs are waking up stiff, tingling in the arm or a neck that tilts when you lie down. Our guide to how to tell if your pillow is the wrong height lists them. For a general overview, read how to choose a pillow for your sleep position and mattress.
How does Pillowise approach it?
We measure instead of guessing. A provider takes three measurements:
- Shoulder width
- Neck circumference
- Neck height
We add your sleeping position (side, back or stomach) and your mattress firmness (soft, medium or firm). The Pillowise algorithm combines these five factors and picks one of 7 sizes. The system indicates the right size for about 95% of people. Weight is not part of it, and the provider checks your position on the pillow and adjusts if needed. More on why we measure is on why a fitted pillow.
The standard pillow is 22 x 14 in. The core is 100% Dutch memory foam, non-toxic and latex-free. The cover is removable and machine washable at 104°F. You also get a 30-day comfort guarantee, after giving the pillow 7-10 days to adjust, and a 5-year warranty on material and production faults.
