Best Pillow for Neck Pain: What to Check Before You Choose
The best pillow for neck pain is the one whose height keeps your neck in a neutral line with your spine in the position you actually sleep in. A pillow does not treat neck pain, but one that is too high or too low can hold the neck bent for hours. Because the right height depends on your shoulder width, sleeping position, mattress and the firmness of the pillow itself, no single brand or shape is best for everyone.
Key takeaways
- Check four things instead of comparing brands: shoulder width, sleeping position, mattress firmness and how firm the pillow foam is under your head.
- A pillow supports a neutral neck position. It is an adjunct to care, not a treatment for a neck condition.
- Pain that is new, severe, getting worse or paired with numbness or weakness needs your own provider, not a new pillow.
What can a pillow do for a sore neck?
A pillow fills the space between your head and the mattress. If it fills that space well, the neck stays close to its natural position while you sleep. If it fills too little or too much, the neck tilts toward the mattress or away from it, and the muscles on one side work overtime for hours.
That is the whole job. It does not correct a disc problem, replace exercise or release a tight muscle. Your chiropractor, physical therapist or massage therapist does that work in the office. The pillow is the part of the picture you bring home every night. For the research on adjusting pillow height, see our overview of clinical studies on adjusting pillow height for neck pain.
What should you check when choosing a pillow for neck pain?
Compare these four factors. Together they decide the height you need.
| What to check | What to look at | Why it matters |
|---|---|---|
| Shoulder width | The distance across your shoulders, especially if you sleep on your side | It sets the gap the pillow has to fill |
| Sleeping position | The position you fall asleep in: side, back or stomach | Each position needs a different height |
| Mattress firmness | Soft, medium or firm, including memory foam and hybrid | It changes how far your shoulder and head sink in |
| Foam firmness | How much the pillow compresses under your head | The height you measure in your hand is not the height you sleep at |
Does shoulder width matter for pillow height?
Yes, most of all for side sleepers. On your side, the shoulder is the widest point between your head and the mattress. A broad-shouldered person needs more height than a narrow-shouldered person to keep the head level. A stock pillow has one height, so it fits only part of the population.
In a pilot EMG study of 15 adults, a pillow height matched to shoulder width produced the lowest neck and shoulder muscle activity during side sleeping (Jiao et al., 2025). The study was small and tested mostly healthy adults, so it shows an association with muscle activity, not a result for people with neck pain. We cover it in detail in side sleeping, shoulder width and pillow height.
Does your sleeping position change the pillow you need?
It does. Here is how the three positions differ in general terms:
- Side sleepers need enough height to fill the shoulder gap, so the nose stays in line with the middle of the chest.
- Back sleepers have a smaller gap. A pillow that is too high pushes the chin toward the chest.
- Stomach sleepers turn the head to one side for hours. That position is hard on the neck for many people, and a low pillow is usually the least stressful option.
If you change positions during the night, use the position you fall asleep in. If you are not sure, choose side. Our post on choosing a pillow by sleep position and mattress goes further.
How does mattress firmness affect the right pillow?
A soft mattress lets your shoulder sink in, which shrinks the gap between head and mattress. A firm mattress keeps the shoulder up, so the gap stays larger. The same pillow can therefore feel right on one mattress and wrong on another. Your body weight matters too. A heavier person sinks deeper and may need a lower pillow.
If you do not know your mattress firmness, medium is a sensible starting point. That applies to memory foam mattresses and hotel beds as well.
Does the firmness of the foam matter?
Yes, because a pillow compresses once your head rests on it. A very soft pillow can flatten to far less than its shelf height. That leaves the neck low, especially on your side. A very firm pillow keeps its height but may not let the head settle, which can feel like sleeping on a block.
Memory foam adapts to the shape of your head and neck. Pillowise memory foam adapts quickly and stays soft at lower temperatures. Whatever you choose, judge the pillow by how your neck sits after you have lain on it, not by how it looks on a shelf.
Can a pillow fix neck pain?
No. Neck pain has many causes, including posture, desk work, stress, joint and muscle problems, and poor sleep itself. A well-fitted pillow removes one avoidable strain from the night. It may make mornings more comfortable for some people, but it is not a cure and we do not promise results. When should you see a provider instead of buying a pillow?
Be seen by a licensed provider if your neck pain follows an injury, is severe or getting worse, wakes you every night, or comes with numbness, tingling, weakness in an arm or hand, or fever. A new pillow is not the answer to those symptoms. If your pain is mild and mostly shows up in the morning, a pillow check is a reasonable thing to add to your care. Our guide to a cervical pillow explains what that kind of pillow is designed to do.
How does Pillowise approach the fit?
We treat pillow size like shoe size. A Pillowise fitting uses three measurements: shoulder width, neck circumference and neck height. It adds your sleeping position and mattress firmness. The Pillowise algorithm then picks one of 7 sizes. Two people with very different bodies can end up with the same size, because five factors are combined.
Weight is not part of the calculation, so the provider checks how you lie on the pillow and adjusts if needed, for example during pregnancy. The core is premium Dutch memory foam, made in the Netherlands, non-toxic and latex-free. The cover is removable and machine washable at 104°F. You can read what happens during the appointment in what happens in a pillow fitting.
Give any new pillow 7 to 10 days to adjust. If it still does not feel right, our 30-day comfort guarantee lets you exchange it for another size or return it for a refund within 30 days of purchase. Return shipping is paid by the customer.
