Custom Pillow: What It Means and How to Get a Real Fit
A custom pillow is a pillow chosen for your body instead of an average one. It only deserves the name if its height is matched to your shoulder width, neck, sleeping position and mattress, because those factors decide how high the pillow needs to be.
Key takeaways
- "Custom" is used for three different things: printed or monogrammed pillows, pillows with adjustable fill, and pillows sized from body measurements. Only the last two address fit, and only the last starts from your measurements.
- Pillow height depends on shoulder width, neck size, sleeping position and mattress firmness. One stock height cannot suit every body, in the same way one shoe size cannot.
- Pillowise picks one of 7 sizes from three measurements plus sleeping position and mattress firmness. The measurement is free at a provider near you.
What does "custom pillow" actually mean?
Search results for "custom pillow" mix very different products. It helps to sort them by what is being customized.
| Type | What is customized | Does it address fit? |
|---|---|---|
| Personalized | Print, photo, name or color | No. Looks only. |
| Adjustable fill | You add or remove fill by trial and error | Partly. You do the guessing. |
| Measured fit | A size is chosen from your body measurements | Yes. This is the idea behind a fitted pillow. |
If your goal is a pillow that suits your neck and shoulders, look at the third row. We explain the thinking in more detail on why a custom pillow.
Why does one pillow height not fit everyone?
The pillow fills the gap between your head and the mattress. That gap is different for every person and every position.
- Side sleepers: the shoulder is the widest point, so the gap is largest. A pillow that is too low lets the head tilt toward the mattress. One that is too high tilts it away.
- Back sleepers: the gap is much smaller, so the pillow usually needs to be lower.
- Stomach sleepers: the head is turned, and a thick pillow tends to push the neck into extension.
Mattress firmness changes the gap too. On a soft or memory foam mattress, the shoulder sinks in and the pillow needs to be lower than on a firm one. For a position-by-position walkthrough, see our guide to choosing a pillow by sleep position and mattress.
What does the research say about matching height?
The evidence is early. In a pilot EMG study of 15 adults, a pillow height matched to shoulder width went along with lower neck and shoulder muscle activity and higher comfort during side sleeping (Jiao et al., 2025). We cover that study in side sleeping, shoulder width and pillow height. Studies on adjusting pillow height for neck pain are summarized in this review of clinical studies.
Scope
These studies are small and mostly involve healthy adults. They show an association between matched height and muscle activity or comfort. They do not show that a fitted pillow treats neck pain or a medical condition. We position a fitted pillow as an adjunct to care, not a treatment.
What should a custom pillow be based on?
A pillow that claims to be made for you should ask for more than "side, back or stomach." The Pillowise fitting combines five factors:
- Shoulder width. Sets the gap for side sleeping.
- Neck circumference. Measured at C4/C5, below the Adam's apple for men.
- Neck height. Measured from the mastoid process diagonally to the outer edge of the acromion.
- Sleeping position. The position in which you fall asleep. If you have no idea, choose side.
- Mattress firmness. Soft, medium or firm. If you do not know, choose medium. That also applies to memory foam mattresses and hotels.
Because five factors are combined, two people with very different bodies can end up with the same size. Weight is not one of the factors. A heavier person sinks deeper into the mattress and may need a lower pillow, so a provider should check how your head and neck sit on the pillow and adjust if needed. Pregnancy is one example where that check matters.
Can I measure myself at home?
You can get an idea. There are two ways to measure: the PillowScan on your phone, or the manual measurement on our page Find your pillow size. Ideally a second person takes the measurements, because the neck points are hard to find on yourself.
A provider does this every day and checks the result on the pillow itself. You can read what happens in a visit in what happens in a pillow fitting.
How does Pillowise approach a custom pillow?
Thijs van der Hilst, a Dutch physiotherapist, started measuring his clients to choose their pillow in 2003. Pillowise was founded in 2013 with United Comfort Industries. The Pillowise algorithm picks one of 7 sizes from the five factors above. The system indicates the right size for about 95% of people.
- Core: 100% premium Dutch memory foam, non-toxic and latex-free, made in the Netherlands. The foam adapts quickly and stays soft at lower temperatures.
- Standard pillow: 22 x 14 in.
- Cover: Tencel top layer on polyester (40% Tencel, 60% polyester), removable and machine washable at 104°F (40°C).
- Comfort guarantee: give the pillow 7 to 10 days to adjust. Within 30 days of purchase you can exchange it for another size or return it for a refund. Return shipping is paid by you, and shipping charges are not refunded.
- Warranty: 5 years on material and production faults. Normal changes in firmness, wear and tear and improper use are not covered.

When will a custom pillow not help?
A pillow is one part of your sleep setup. It will not fix a worn-out mattress, and it cannot replace an assessment. See your own chiropractor, physical therapist or physician if you have neck or arm pain that is getting worse, numbness or weakness, pain after an injury, or morning stiffness that does not ease. If you already suspect your current pillow is the problem, our checklist on how to tell if your pillow is the wrong height is a good first step.
Find a provider near you
Pillowise is sold only through licensed healthcare providers in the US: chiropractors, physical therapists and licensed massage therapists. It is not sold in retail stores or through a direct webshop. You find a provider, make an appointment, and the measurement is free at every reseller. The provider takes the measurements, and you take the fit home.
If you use an FSA or HSA, ask your plan administrator whether a pillow qualifies. Questions about the process are answered in our FAQ.
