Best Pillow for Back Sleepers: Why Fit Matters More Than Fill
The best pillow for a back sleeper is one whose height fills the space under the neck and keeps the head level, not tipped forward and not tipped back. That height differs from person to person, so a pillow chosen from your measurements usually fits better than a stock "medium" one.
Key takeaways
- A back sleeper needs a pillow that supports the curve of the neck and keeps the head level. Too high pushes the chin toward the chest. Too low lets the head tip back.
- Neck size, mattress firmness and body build all change the height you need. Weight alone does not decide it.
- A Pillowise fitting takes three measurements (shoulder width, neck circumference and neck height), adds sleeping position and mattress firmness, and picks one of 7 sizes. The measurement is free at a provider near you.
What does a pillow have to do for a back sleeper?
On your back, the head rests on the pillow and the neck sits in the gap between the head and the mattress. The pillow has to fill that gap. If it does not, the neck spends the night bent forward or backward.
Side sleepers have to bridge the width of the shoulder. Back sleepers have a different job: the pillow has to match the shape and size of the neck. That is why neck circumference and neck height matter so much in a back-sleeper fitting.
Is a thick or a thin pillow better for back sleeping?
Neither is better for everyone. The right pillow is the one that matches your neck. These signs can help you judge what you have now:
| Sign | Pillow may be too high | Pillow may be too low |
|---|---|---|
| Head position | Chin tipped toward the chest | Head tipped back, chin pointing up |
| Gap under the neck | Neck feels pushed forward | Neck feels unsupported, with a hollow under it |
| On waking | Stiff or tight at the back of the neck | Stiff or tight at the front or base of the neck |
These signs are a rough guide, not a diagnosis. For a longer checklist, see how to tell if your pillow is the wrong height.
Does my mattress change the pillow I need?
Yes. On a soft mattress, including many memory foam and hybrid models, the body sinks in more. A heavier person sinks deeper still and may need a lower pillow. On a firm mattress the head and shoulders stay higher, so the pillow has to make up a different distance.
This is why we ask for mattress firmness (soft, medium or firm) in every fitting. If you do not know, choose medium. Our post on choosing a pillow by sleep position and mattress covers this in more detail.
Which material works for back sleepers?
Material matters less than height, but it does affect comfort. A foam that adapts to the head and neck can support the curve under the neck. Pillowise pillows use a core of 100% premium Dutch memory foam that is non-toxic and latex-free. The foam adapts quickly and stays soft at lower temperatures. The removable cover is 40% Tencel and 60% polyester and is machine washable at 104°F (40°C).
Why does a fitted pillow make sense for back sleepers?
One stock height cannot suit every neck, shoulder width and sleeping position. That is the idea behind why a custom-fit pillow makes sense: like a shoe size, a pillow should fit the person using it. With Pillowise, "custom-fit" means choosing one of 7 sizes based on your measurements. It does not mean a pillow made from scratch for you.
How does Pillowise pick a pillow for a back sleeper?
- Measure the body. Three measurements: shoulder width, neck circumference and neck height. Neck height runs from the mastoid process diagonally to the outer edge of the acromion. Ideally a second person takes the measurements.
- Add your sleeping position. This is the position in which you fall asleep. If you fall asleep on your back, choose back.
- Add your mattress firmness. Soft, medium or firm. Not sure? Choose medium.
- Get the size. The Pillowise algorithm combines the five factors and selects one of 7 sizes. Two people with very different bodies can end up with the same size.
- Check the position. The provider checks how you lie on the pillow and adjusts if needed.
The system indicates the right size for about 95% of people. Weight is not part of the calculation, which is why the provider's check of your position on the pillow matters. You can also try the manual measurement on our size page. To see what happens at an appointment, read what happens in a pillow fitting.
What does the research say?
We have not cited a study of back sleepers specifically on this page. The closest pillow-height study in our blog is a pilot EMG study of 15 adults. It found that a pillow height matched to shoulder width produced the lowest neck and shoulder muscle activity during side sleeping (Jiao et al., 2025). That study looked at side sleeping, so it does not answer the back-sleeper question directly. Studies of this size tell us about muscle activity and comfort in a small group. They do not show that a pillow treats a neck or back condition. We see a fitted pillow as an adjunct to care, not a treatment. For more, see our summary of pillow height and neck pain studies.
When will a pillow not help?
A pillow cannot fix everything. See your own provider if you have neck pain after an injury, pain that wakes you at night or does not ease with rest, or numbness, tingling or weakness in the arms or hands. A fitted pillow can be one part of the plan once the cause is assessed.
What if I fall asleep on my back but turn over at night?
Use the position in which you fall asleep. If you have no idea, choose side. If you spend much of the night on your back, mention that to your provider at the fitting so they can check the pillow in that position too.
What if the pillow does not feel right?
Give a new pillow 7-10 days to adjust. After that, you can exchange it for another size or return it for a refund within 30 days of purchase under the 30-day comfort guarantee. Return shipping is paid by the customer, and shipping charges are not refunded. There is also a 5-year warranty on material and production faults. If you want to use an FSA or HSA, ask your plan administrator whether it applies.
