Cervical Pillow for Neck Pain: What It Can and Cannot Do

    A cervical pillow can support a neutral neck position at night, but only when its height fits your shoulder width, neck and sleeping position, because a pillow that is too high or too low keeps the neck tilted for hours. It is an adjunct to care, not a treatment, and it does not replace an exam when pain is severe or not improving.

    Key takeaways

    • Height matters more than the label "cervical." A contoured pillow in the wrong height can hold your neck out of line as easily as a flat one.
    • Shoulder width, neck size, neck height, sleeping position and mattress firmness all change which height fits. One stock height cannot suit everyone.
    • A pillow will not fix every kind of neck pain. Severe pain, pain after an injury, or numbness and weakness in the arms or hands call for an exam by your own provider.

    What is a cervical pillow?

    "Cervical pillow" is a general term for a pillow designed to support the cervical spine, the seven vertebrae of the neck. Most have a shaped surface, with a curve under the neck and a lower area for the head. Some are made of memory foam, others of latex, fiber or water-based fill.

    The term is not regulated, so the shape alone tells you little. What matters is whether the pillow fills the space between your head and the mattress at the height your body needs. For a closer look at why fit matters for neck complaints, see our post on why proper pillow fit matters for neck pain.

    Why does pillow height matter for a sore neck?

    Sleep is a long, static posture. If the pillow is too low, the head drops toward the mattress. If it is too high, the head is pushed up and to the side. Either way, the neck muscles and joints stay in a tilted position for hours, and many people wake up stiff.

    The gap to fill is different for every person. It depends on:

    • Shoulder width. A wider shoulder lifts a side sleeper's head further from the mattress. We cover this in side sleeping, shoulder width and pillow height.
    • Neck circumference and neck height. These describe the size of the neck itself and how it sits between the head and the shoulder.
    • Sleeping position. Side, back and stomach sleepers need different heights. We use the position you fall asleep in. If you are not sure, choose side.
    • Mattress firmness. On a soft mattress your shoulder sinks in and the gap shrinks. On a firm one it stays large.

    This is why a pillow that works well for your partner can feel wrong to you. It is the same reason a shoe size that fits one person does not fit another.

    What can a cervical pillow do, and what can't it do?

    A well-fitted pillow can remove one avoidable source of strain: a neck held out of line all night. That may make mornings more comfortable for some people. It does not change the cause of a neck problem.

    A pillow cannot:

    • diagnose or treat a neck condition such as a disc problem or a pinched nerve;
    • replace exercise, manual care or other treatment your provider recommends;
    • help with pain that comes from daytime posture, stress or a recent injury without those being addressed too.

    For the clinical studies on pillow height and neck pain, and what they do and do not show, read adjusting pillow height for neck pain: clinical studies. Many studies in this area are small and include healthy adults, so we treat the evidence with caution.

    When should you see a provider instead of buying a pillow?

    See your own provider, or a physician, if you have:

    • severe neck pain, or pain that is not improving;
    • neck pain after a fall, accident or other injury;
    • numbness, tingling or weakness in your arms or hands;
    • fever, or pain with other symptoms that worry you.

    A pillow is a sensible step once those questions have been answered, not a way to skip them.

    How do you know if a pillow is the wrong height?

    Common signs are waking with a stiff neck, switching hands under the pillow, stacking or folding it, or needing to adjust it again and again at night. We list more in how to tell if your pillow is the wrong height. These signs suggest the height may be off. They do not prove it, since mattress, stress and daytime habits also play a role.

    How does Pillowise approach a cervical pillow?

    We do not guess at height. Pillowise chooses one of 7 sizes from three measurements plus two choices:

    InputWhat it describes
    Shoulder widthThe distance the pillow has to bridge for a side sleeper
    Neck circumferenceMeasured at C4/C5 (below the Adam's apple for men)
    Neck heightFrom the mastoid process diagonally to the outer edge of the acromion
    Sleeping positionSide, back or stomach, the position you fall asleep in
    Mattress firmnessSoft, medium or firm. If you do not know, choose medium

    The Pillowise algorithm combines these five factors. Two people with very different bodies can end up with the same size. The system indicates the right size for about 95% of people. Weight is not part of the calculation, so a heavier person who sinks deeper into the mattress may need a lower pillow, and the provider checks your position on the pillow and adjusts if needed, for example in pregnancy.

    The standard pillow measures 22 x 14 in. The core is 100% premium Dutch memory foam, non-toxic and latex-free, made in the Netherlands. The removable cover is 40% Tencel and 60% polyester and is machine washable at 104°F (40°C). You can read about the appointment itself in what happens in a pillow fitting, and see how the sizes work on our pillows page.

    Give a new pillow 7 to 10 days to adjust. Within 30 days of purchase you can exchange it for another size or return it for a refund. Return shipping is paid by the customer, and shipping charges are not refunded. There is also a 5-year warranty on material and production faults. Details are in our FAQ.

    Pillowise is sold only through licensed healthcare providers in the US: chiropractors, physical therapists and licensed massage therapists. It is not sold in retail stores or on a consumer webshop. The provider takes your measurements, and you take the fit home. The measurement is free at every reseller. If you plan to use FSA or HSA funds, ask your plan administrator whether a pillow qualifies.

    If you want to see how the measurements work first, our size guide walks through all three. Ideally a second person takes them for you.
